Key Responsibilities
- Develop and deliver diabetes education programs for patients and their families
- Coordinate individualized care plans in collaboration with endocrinology and nursing teams
- Monitor patient progress and adjust education strategies based on clinical outcomes
- Lead group education sessions on diabetes self-management and lifestyle modifications
- Support newly diagnosed patients with comprehensive onboarding education
- Maintain accurate documentation and patient records in compliance with hospital standards
- Promote awareness campaigns related to diabetes prevention and management
- Contribute to quality improvement initiatives within the diabetes care department
- Collaborate with multidisciplinary teams to ensure holistic patient care
Requirements
- Bachelor’s degree in Nursing or a related healthcare field
- Valid DHA license or eligibility as a Diabetic Educator or Registered Nurse
- Certified Diabetes Educator (CDE) credential preferred
- Minimum 3 years of experience in diabetes education or endocrinology nursing
- Excellent communication and patient counseling skills
- Strong knowledge of current diabetes management guidelines and technologies
- Proficiency in electronic health records and MS Office applications
