Responsibilities of Nursing Case Manager

• The role of the case manager is to ensure a smooth and wholesome patient safety centric approach from the point of admission of the patient up until discharge.
• Ensuring the appropriate level of care is implemented in line with the plan of care.
• The role would also perform the necessary advocacy on the services and plans formulated during the stay.
• Adheres to the insurance approvals and advocates the patients’ requirements to the respective hospital stakeholders.
• The role is responsible to assess each patient’s psychosocial and economic situation as it relates to their health status, progress along the continuum of care, return to wellness and safe/timely discharge.

Qualifications & Requirements:

Following are the requirements to be eligible for the role of Nursing Internal Case Manager at SRH:

• Bachelor’s Degree in Nursing from an accredited nursing school.
• Master’s Degree or post graduate study is preferable.
• 2 years Case Manager/ Case Coordinator/ Discharge Coordinator experience in a Healthcare Setting.
• 3 years Clinical Nursing experience.
• DOH License as Registered Nurse is a must.
• Commission for Case Manager Certification, or equivalent is highly desirable.

Required Skills

• Effective English communication skills (written & verbal).
• Ability to create and prepare reports and dashboard.
• Multitasking and time management skills.
