Responsibilities of Assistant Social Worker

• Social Worker is responsible for psychosocial assessment, counseling, crisis intervention/management, daily treatment team meetings and daily group therapy sessions to the patients and families.
• Provides patients with a formal after-care plan and linkage with community resources within a multi-county area.
• Works collaboratively with community agencies, other facilities, medical staff, and the multidisciplinary team to effect positive outcomes.
• Conducts initial screening and evaluation of patients.
• Conducts patient psychosocial assessment and makes treatment recommendations.
• Provides consultation and collaborates with physician’s nurses and other health care team members to assist in the development of a comprehensive and coordinated plan of care
• Engages patients and their families in adhering to the treatment plan by reducing or eliminating, to the extent possible, social, psychological, financial and behavioral barriers to successful outcomes.
• Assists Case Manager with complex social situations.
• Coordinate with Case Manager with patient discharge and continuity of care following discharge.
• Acts as an advocate for patients and families – including as an advocate for the patient’s health care rights.
• Educates patients on the roles of other members on their recovery team including physicians, nurses, physical therapists, etc.
• Aids and expedites decision-making on behalf of patients and their families.
• Advocates on the patient’s behalf for access to appropriate post- acute care services.
• Identifies and assists the under or uninsured patient to locate available resources and advocates with payers and social agencies to locate sources of financial support
• Acts as a resource to SRH personnel on a wide range of social and cultural topics.
• MmkAdvocates in the provision of emotional support and education for staff.
• Engages patients and families following discharge process provide support and community integration.

Qualifications & Experience

Given below is the eligibility criteria for the role of Assistant Social Worker (DOH Licensed) at SRH:

• An associate / Bachelor degree in social work, Nursing, social sciences, human services, or related.
• 1-2 years experience in social work.
